Volunteer Forms
Complete and return the following forms if you would like to volunteer:
- Driver Information Form
- Volunteer Data Sheet
- Adventist Screening Instructions (Print the certificate at the end of the online course)
- CACS Volunteer Interest Form
Return all forms and required documentation to the principal (she will submit the forms to the school board for approval).
